4. Go back to the home page.

5. On the bottom of the page click the "Privacy Policy" link.

6. On the bottom of the Privacy Policy page, click "HERE".

7. Scroll down to the form, enter your Telephone number and 5-digit ZIP code.

8. Click the "Request Removal" button.

9. Success! You have submitted your removal request successfully.
