1. Go to www.revealname.com.

2. Enter your phone number. Click the "Name Lookup" button.

3. Complete the captcha

4. When you find your record, copy the link to your profile.

5. Open the page below in a new tab. Paste the link, complete the captcha and click the "remove my info" button.

6. Success! You have submitted your removal request successfully.
